So how does this guy even climb onto the boat with that thing? Makes me queasy just looking at it.

Did you notice the life jacket he still has around him? He was probably hauled out and onto the CG ship.

I think he has a life jacket on, but also there are straps the board he is on to keep him immobile as well.
Looks like he didn't get into the water and the accident happened while still in the boat. Shirt and those shorts he is wearing are dry, they have been cut back by the EMS, not a wetsuit you would have on if your were diving and spearfishing.
Probably just an unlucky passenger.
